African American Rhinoplasty ProcedureAfrican noses are considered platyrrhine in shape, which is characterized by thicker skin, weaker cartilages, broader nasal base and bridge, and, of course, darker skin. Today’s watchword is ethnic preservation rather than ethnic effacement. As a true artist, Dr. Lam’s focus while performing nose surgery is on enhancing your appearance while preserving your ethnicity. Radical changes to a nose intended to erase one’s race does not look natural and should be avoided by the judicious and tasteful surgeon. Softening features that are well balanced to the face should be the goal in every case. Dr. Lam works very hard to make the face look not only appropriate for one’s ethnicity and gender but also one’s facial features. During consultation, he will work with you to achieve the most natural outcome that is mutually agreed upon through a careful conversation and 3D imaging to ensure mutual collaboration is properly established. Starting with the nasal bridge, Dr. Lam may or may not narrow the nasal bridge because sometimes when the nasal bridge is overly narrowed it can make the nasal tip appear too large. Total nasal balance is critical for an aesthetically pleasing nasal result. Instead, Dr. Lam may raise the nasal bridge in certain cases since the African nose tends to be flat and broader. By raising the bridge, the nose may appear narrow and more refined and balance the nasal tip. Occasionally, he will perform both nasal bone narrowing along with raising the nasal bridge. He performs nasal bridge elevation using diced cartilage with tissue glue to achieve the most natural results, as he no longer uses any kind of artificial implants that are prone to infection, compression, warping, and visible edges. Dr. Lam may also reduce a nasal hump and combine many of the above techniques depending on your situation. For the nasal tip, Dr. Lam believes that over reducing a broad nasal tip will not only look unnatural but also may cause functional issues. In addition, over reducing a broad nasal tip may cause the thick skin not to settle down and make the nasal tip actually appear larger through the development of internal scar tissue. He uses multiple techniques to refine the nasal-tip shape without causing these issues. He believes in strengthening the nasal tip rather than weakening it through reductive maneuvers. With the nasal base, or nostril rims, Dr. Lam is focused on minimal scarring and a retaining a natural curvature to the nostril shape. He believes the cut off appearance of the nostril should not be the case when reducing a nostril size. He uses a combination of techniques to avoid this issue that are described more thoroughly in the technique section of this website.
